Phyllis L. Chitwood, age 68 of Beaver Dam, passed away unexpectedly on Sunday, December 16, 2012 at her home in Beaver Dam.

Phyllis was born October 21, 1944 in Beaver Dam to Albert Lentz and Rose (nee Annis) Lentz. She is survived by her loving children, Julian Chitwood, Alvin Charles Chitwood, Grace Chitwood, Jelona Williams and Erma (Ricardo) Garrido; grandchildren, Julian Christopher Chitwood, Ryan Chitwood, Brinkley Chitwood, Peyton Chitwood, Kayla Chitwood, Jennifer Schoenfeld, Jessica Schoenfeld, Jamie Schoenfeld, Eric Wellnitz, Dustin Wellnitz, Nakeya Wellnitz, Shane Jr. "Fat Boy" Wellnitz, Lucia Chitwood, Anastasia Garrido, Aishah Garrido, and Alejandro Garrido. Phyllis is also survived by her siblings, Meredith Lewis, Dennis Lentz, William (Becky) Lentz, Michael Lentz, Karen (Bill) Fritz, a very special cousin, Gloria (Tony) Hildebrandt; and favorite nephew, Joshua Lentz with many more loving nieces, nephews and friends.

Phyllis is preceded in death by her parents, and her siblings, Hubert Lentz, Henrietta Mache; and joined in death by Sharen Hansen who passed away on the same day only a few hours apart.

A Memorial Service for Phyllis will be held on Wednesday, December 19, 2012 at 5:00 p.m. at Cornerstone Funeral & Cremation Services, 135 Keller Blvd. in Beaver Dam. Visitation will also take place on Wednesday at Cornerstone 1 hour prior to the service. An inurnment will take place at a later date.
